Registration for students attending as part of a school group
Dear Parent/Guardian,
Please complete the following form to register your child to take part in the COBIS GSL Leadership Weekend. You will receive all the emails that are sent to your child as part of the programme.
You must complete the parental consent form as soon as possible and no later than 31st October when registration closes. If you do not complete the parental consent form your child will not receive the log in details for the programme.
Programme Consent
By completing this form you are agreeing to give consent as follows:

- I consent to my child attending this programme, including the Live Online Sessions.
- I give permission to the organisers of this event to contact me and/or my child with further details of the event by email.
- I have read the Code of Conduct and will make sure that my child behaves appropriately and responsibly and follows the Code of Conduct that they have signed.
- I understand that if my child does not follow the Code of Conduct, they will be removed from participating in the remainder of the programme at the sole discretion of Future Foundations.
